By completing your registration for the Utah E.A.S.Y. Off-Premise Alcohol Sales Training course on dashauthority.com, you enter into this Enrollment Agreement with Dash Authority LLC (“Provider”). This agreement meets all requirements set by the Utah Division of Substance Abuse and Mental Health (SUMH) for approved E.A.S.Y. training providers.
1. Obligations of the Provider
- We will issue your official certificate of completion and submit your training record to the Utah E.A.S.Y. state registry.
- We will provide immediate access to download/print your certificate upon successful completion of the course and passing the final exam.
- We will enter your completion into the state system so employers can verify it instantly.
- We maintain at least one SUMH-approved trainer available during posted business hours to answer content or technical questions.
2. Obligations of the Student
- You must complete the entire training course (all modules and quizzes) before you are allowed to take the final exam.
- You must pass the final exam with a minimum score of 70%.
- You must provide complete and accurate information during enrollment, including your full legal name, date of birth, last four digits of your Social Security Number (or written confirmation that you do not have one), and email address. This information is required for entry into the Utah E.A.S.Y. state system.
- Your E.A.S.Y. certificate will not be issued or made available until the full course and exam are successfully completed.
3. Refund Policy
- Full refund (100%) if you request cancellation before accessing any course content or modules.
- No refunds once you have started the course (i.e., accessed any lesson, quiz, or module).
- Refunds, when eligible, will be processed to the original payment method within 5–7 business days.
- Technical issues preventing course access may qualify for a full refund or free course transfer at our discretion.
4. Steps to Terminate Enrollment / Cancel
- Contact us immediately by email at [email protected] or by phone at 202-335-2298.
- Provide your full name, email used for enrollment, and reason for cancellation.
- If you have not yet started the course, we will cancel your enrollment and process your refund (if applicable).
- Once the course has started, cancellation is no longer possible and no refund will be issued.

6. Additional Important Notice
Students must complete the entire training course before taking the final exam. You may not skip ahead, and the course is designed with required student interaction at regular intervals. The final exam follows SUMH standards.
By enrolling, you acknowledge that:
- You are on a third-party website (dashauthority.com). This course is SUMH-approved but is not operated by the Utah Division of Substance Abuse and Mental Health or the Department of Alcoholic Beverage Services.
- You have read and agree to the full Privacy Policy and Technical Standards.
- The last four digits of your Social Security Number are provided to the state system and will be permanently removed from our records after submission.
- All course materials are copyright protected and for your personal use only.
